Selected List of Books and Journals for the Small Medical Library* BY ALFRED N. BRANDON, Librarian The New York Academy of Medicine New York, New York ABSTRACT This revised list of 446 books and 137 journals is intended as a selection guide for small or mediumsized hospital libraries or for the small medical library serving a specified clientele. Books and journals are categorized by subject, with the books being followed by an author index and the journals by an alphabetical title listing. Items suggested for initial purchase by smaller libraries are indicated by an asterisk. To purchase the entire collection of books and to pay for annual subscriptions to all the journals would require an expenditure of about $14,500. The cost of only the asterisked items recommended for first purchase totals approximately $4,100.
DURING the past decade an increasing emphasis has been placed on the clinical and educational impact of the small medical library within its own institution and in the overall biomedical library community as well. The National Library of Medicine Regional Medical Library Program has been an effective force in upgrading both the role and resources of the small medical library. In addition, the RMLP has provided a structured means for the inclusion of this type of library as an integral part of a national biomedical communications network program. In order to carry out its function in the RMLP concept, the hospital library must have an up-to-date basic book and journal collection that will meet the immediate needs of its primary clientele. However, in the foreseeable future, the demands made on the small medical library will entail even greater responsibilities because the essentiality of life-long continuing education for the practicing physician has reached a new level of consciousness within the medical * A sixth revised version of a paper originally published in the Bull. community. This premise is gaining and undoubtedly will continue to gain wide acceptance, thereby substantially increasing the immediate needs of the primary clientele of the hospital library-needs that cannot be met totally by a traditional basic book and journal collection. In view of financial problems faced by health-care facilities and lack of space in many institutions, it is unrealistic for every small hospital to amass a large library proportional to the ever-increasing requirements of its medical staff. Nonetheless, irrespective of size, providing prompt access to biomedical information for the health-care professionals of its institution is a mandatory function of any hospital library. At the present time, this mandate can be carried out most effectively and economically by ventures in resource sharing which can range from informal agreements between libraries in two neighboring hospitals to multi-institutional library consortia formed on the bases of geographical location and/or subject interests. As the clinical and educational aspects of the hospital library's role increase while institutional fiscal austerity increases at a comparable rate, the selection of books and journals becomes more complex and difficult for the hospital librarian. The Stearns-Ratcliff "core library" (1), published in 1970, was conceived as the basic collection necessary for a hospital library. However, its scope is minimal and restricted; today it would hardly meet the needs of even the smallest hospital with any degree of adequacy. As yet, the "core library" list has not been revised although there are tentative plans to update it on a continuing basis eventually. Many core lists have been compiled by the Regional Medical Libraries and by the Regional Medical Programs. The list of books and journals presented here, which is revised biennially, is meant primarily

to serve as a guideline in establishing and maintaining a substantive small medical library collection, but additionally, its purpose is also to supplement core lists and to complement more inclusive selection aids. The book section of the present version has been enlarged to allow for greater selectivity within a subject field for hospital library consortia engaged in cooperative acquisitions or for the teaching hospital library. The list has been totally reevaluated in accordance with recommendations made by both subject specialists and medical librarians; many of their suggestions have been incorporated into it. The many hours of work by Dorothy R. Hill, Assistant Professor of Library Science and Acquisitions Librarian, Mount Sinai School of Medicine of The City University of New York, coupled with her exceptional knowledge of medical literature and medical librarianship, have been invaluable in the preparation of the current revision. Even though this list is basically intended to be a selection aid for hospital librarians, previously published versions have been used extensively by medical and academic librarians, by physicians for personal book selection, and in teaching programs. If an institution were to purchase all the books and journals in this list, the approximate costs would be $10,400 for books and $4,100 for the annual journal subscriptions, making a grand total of $14,500. Because it is impossible for many institutions to buy all, or in some cases, even most of the listed books and journals, items suggested for priority purchase are indicated by an asterisk. To acquire only the asterisked books and journals, approxi-
mately $4,100 would be needed-$2,700 for the books and $1,400 for journal subscriptions for one year. Since the inception of this list in 1965, book and journal prices have continued to rise steadily (Table 1). The average cost of a journal subscription has increased 113.7% from 1965 ($13.90) to 1975 ($29.71). The average price per book has jumped from $15.00 in 1965, to $23.33 in 1975, a 55.5% rise in cost. The figures reflected in Table 1 are based exclusively on prices for publications that have been and are included in this list. Therefore, it must be recognized that the figures given here are relevant only to the small medical library collection and have no validity when applied to average book and journal prices, which are significantly higher, that confront large medical libraries because of the more expensive materials that make up their collections. Price information in this paper is as accurate as possible at the time of its publication, but if the inflationary spiral continues at its current rate, many of the prices will inflate accordingly within a relatively short time span. There are comparatively few up-to-date selection guides available to the hospital librarian, and for the inexperienced individual, book and journal selection can present an overwhelming dilemma. "A Library for Internists Recommended by the American College of Physicians" (2), which appeared in 1973, is one of the most noteworthy recent contributions made in this field. However, this list of books and journals was compiled specifically for the internist and is geared totally toward internal medicine. It accomplishes its purpose well for

the internist, but for the hospital librarian it provides little help in the selection of books and journals in other fields of clinical medicine and relatively no help in choosing basic science publications. My list which follows has been compiled especially for the librarian and offers direction in the building of a well-balanced small medical library collection that includes both clinical and pertinent basic science materials. The Library Association's Medical Section has compiled a subject listing titled Books and Periodicals for Medical Libraries in Hospitals consisting of ". . . titles which are generally considered most likely to provide useful coverage in the various subject fields. . ." (3); both basic and clinical sciences are included. It must be remembered that this list is British in origin and viewpoint. This factor'should be taken into consideration especially when using it for the selection of books because a preferred medical text in the United Kingdom is not necessarily accepted as such in the United States. The Library Association presents the list only as a selection tool and offers no preference evaluation of the titles it contains. The catalogs compiled by associations, e.g., American Hospital Association, National League for Nursing, American Medical Association, etc., which list their publications, are valuable acquisitions tools for the hospital librarian. Publications of national professional associations are essentially authoritative in scope, generally inexpensive, and tend to be used frequently by hospital library clientele. Their inclusion can greatly enhance even a very small hospital library collection. Other lists which have been cited in earlier versions of this paper that are still current enough to be useful in the building of a hospital library collection today are noted in the "References" (4-8). Outdated selection aids are not included in the current references, but for comparative purposes, their bibliographic data and evaluation can be accessed from the references and text accompanying my previous selected lists (see footnote, p. 149). A wealth of useful information for all hospital librarians has been assembled by the Joint Committee on Library Service in Hospitals, CNLA, in its excellently formatted Basic List of Guides and Information Sources for Professional and Patients' Libraries in Hospitals Bull. (9). This is a unique reference document, which is available free of charge, and all hospital librarians should be familiar with it. Library Practice in Hospitals: A Basic Guide (10) is a book containing fundamental knowledge for the inexperienced hospital librarian and is a worthwhile reference manual for any hospital librarian. Its chapters on selection and acquisition of books and journals contain basic concepts of which the hospital librarian should be cognizant when using any selection aid. More detailed and advanced information can be found in comparable chapters of the Handbook of Medical Library Practice (1 1). Since the National Library of Medicine Regional Medical Library Program has drawn the hospital library and librarian into the mainstream of the biomedical library community, the hospital librarian should call upon and make use of the resources provided by his local Regional Medical Library. A listing of the Regional Medical Libraries can be found in the Basic List of Guides and Information Sources for Professional and Patients' Libraries in Hospitals (9). By contacting the Director of the Regional Medical Library, the hospital librarian can determine if local RML and/or RMP guides and core lists are available and if workshops and seminars on topics affecting hospital medical libraries are sponsored by either one or by both of the programs. An awareness of Regional Medical Library policies is especially important for the hospital librarian at this time. For example, each RML is being requested by the National Library of Medicine to formulate restrictive lists of journals that are not eligible for RML interlibrary loan funding. Included in these lists will undoubtedly be some journal titles that small hospitals do not own. If hospital libraries cannot afford to purchase the restricted journal titles, their access to this material will rely on the innovative efforts on the part of their librarians to set up programs for resource sharing, consortia, and liaison arrangements with larger medical institutions. For hospital libraries, the sharing of expensive basic science journals such as the Journal of Biological Chemistry or the A merican Journal of Physiology, both of which are recommended in this list and whose 1975 subscription prices are $200 and $75 respectively, is the most efficient way to deal with limited budgets and provide a 151

channel for access to these journals. The standard basic science journal literature will become more important to the small medical library as its role in continuing education is further defined and formulated. Without question, access is essential, but this statement in no way indicates that it is necessary or possible for every hospital library individually to subscribe to all the basic science literature that it will be expected to supply to its clientele. In local resource sharing endeavors, hospital librarians should seek out and receive guidance from their Regional Medical Libraries. C. Lee Jones has presented a logical plan for serials resource sharing in his recent article, "A Cooperative Serial Acquisition Program: Thoughts on a Response to Mounting Fiscal Pressures" (12). While his paper was not intended primarily for hospital libraries, many of its principles can be modified to meet the requirements of resource sharing among small medical libraries. The selected list of books and journals set forth here is meant to serve as a selection aid for the small library of a hospital, society, clinic, or similar organization. It should be related and adapted to the activities, objectives, and dominant subject specialties of the particular institution of which the library is a part. If the library is involved in a consortium or a resource sharing program, this, too, should be taken into consideration. Emphasis has been placed on selecting the most recent and authoritative works in the English language. The list is not a complete coverage of all available substantive literature in the subject fields, and therefore, in most libraries, it should be supplemented by additional books and journals in the specialties of greatest interest to the library clientele. Very few expensive sets are included, and less expensive alternatives are indicated within the same subject category. The best selection of reference works can be found in "Selected Reference Aids for Small Medical Libraries" revised by Howertine Farrell Duncan, Reference Librarian, National Library of Medicine (13), and no attempt has been -made for covering them systematically in this list. I have incorporated three highly technical reference subject works into the section on Pharmacology and Therapeutics, namely, The National Formulary, The United States Dispensatory, and The Pharmacopeia of the United States of A merica. In situations where these

titles are normally purchased for the hospital pharmacy or pharmacology department, the value of duplicating them is questionable. If they are not housed in the library, the hospital librarian should know where they are located within the institution and have recourse to them for reference purposes since in the United States, they are generally the most widely recognized reference sources for pharmaceutical standards and for therapeutic drug usage. Again this year, the list of nursing books has been extensively revised and somewhat expanded to cover more aspects of the profession. The number of nursing books recommended is purposely out of proportion to the other subject categories because at the present time, there is no recent comprehensive, recognized selection guide to assist hospital or nursing school librarians in their choice of books in this subject area. My list does not fully meet that need, but the nursing section was compiled with the hospital librarian whose library supports a nursing curriculum or with the inexperienced nursing school librarian in mind. For other small medical libraries, the nursing section may be too detailed, and the asterisked items will serve their purposes. Conversely, it is not detailed enough for a nursing school library, but it does offer a broad base on which to build a collection of nursing books. The Interagency Council on Library Resources for Nursing published a revision of its "Reference Sources for Nursing" in the May 1974 issue of Nursing Outlook (14). Dale E. Shaffer has put together in Library Resources for Nurses ". . . a selected bibliography of basic material and equipment useful in supporting the NURSING curriculum" (1 5). In addition to books and periodicals, pamphlets and audiovisual materials are included in Shaffer's list, making it of special value to the librarian confronted with the problem of building a basic nursing collection not only in the medium of print but also in the audiovisual media as well. Ordering books directly from publishers is, for the most part, an inefficient, expensive procedure for either the hospital librarian or the hospital purchasing agent, since man hours are used counterproductively in generating multiple purchase orders and in handling their resulting invoices which eventually build up to a massive amount of unnecessary paper work.

The practice is now being discouraged by some publishers, especially medical and scientific, who refuse to discount on retail orders even from health-care institutions. A dependable medical book dealer, preferably located in one's own geographic area, serving as a single book supply source, can alleviate much of the paper work and many of the problems encountered when ordering directly from numerous publishers. Usually reliable medical book dealers offer a 10% discount on books from major medical publishers in the United States. Likewise, as a time-saving economy measure, journal subscriptions should be entered through a subscription agent rather than directly with publishers. Even though most agents are currently charging service fees ranging, on the whole, from 5 to 12%, the charge is still considerably less than the expense of labor costs involved in dealing directly with the publishers. A subscription agency is preeminently selling service, and before choosing an agent, an astute librarian will thoroughly investigate the various alternatives while having in mind the standard of service as the ultimate criterion of evaluation. A low service fee does not necessarily mean a monetary saving for a library if the agent's service is substandard. The most valid information regarding a subscription agency can be obtained from medical and academic librarians in the surrounding area who use the services of that agency. An inexperiBull. Med.
