ABSTRACT

This revised list of 472 books and 138 journals is intended as a selection guide for small or medium-sized hospital libraries or for the small medical library serving a specified clientele. It can also be used as a core list by small hospital library consortia. Books and journals are categorized by subject, with the books being followed by an author index and the journals by an alphabetical title listing. Items suggested for initial purchase by smaller libraries are indicated by an asterisk. To purchase the entire collection of books and to pay for annual subscriptions to all the journals would require an expenditure of about $18,200. The cost of only the asterisked items recommended for first purchase totals approximately $4,500.

COLLECTIVELY, the small medical libraries in hospitals and local health facilities throughout the country are now recognized as the foundation support of the Regional Medical Library Network, one of the major parts of the National Library of Medicine's Biomedical Communications Network. The NLM has designated these libraries as "Basic Units" in the hierarchical structure of the RML Network. At local levels, the small medical libraries provide services as necessary as those of the National Library of Medicine on a nationwide level. As a result of the Regional Medical Library Program, librarians in small health science libraries have established their identity in and responsibility to the biomedical community. It is an awesome responsibility, and for small biomedical libraries to carry it out effectively, they must be adequately supported as essential health care entities by their parent institutions. brary materials, and funding continues to remain uncertain, it is becoming more difficult for many hospital libraries to maintain current basic book and journal collections capable of fulfilling even the immediate needs of primary clientele. Yet the mandate of the hospital library is to provide rapid access to biomedical information for the health care personnel of its institution. Because of budgetary constraints, changes in the pattern of federal support for libraries, and massive flows of information from all sources, attempts at autonomous self-sufficiency by any library, regardless of size, are futile in today's changing environment. Cooperative resource sharing offers the only viable, cost-effective alternative for libraries to cope with these problems and at the same time to provide prompt access to health science information. Resource-sharing efforts can extend from an informal agreement between or among libraries in neighboring health care facilities to a formal multi-institutional library consortium, based upon geographical location and/or subject interests, in which the participating institutions have legal and financial commitments to other members in the group. The National Library of Medicine has recognized the need to build up health science libraries at the local (Basic Unit) levels to strengthen the access capabilities of the RML Network as a whole. Two types of Medical Library Resource Improvement Grants are now available from NLM: (1) the Consortium Grant and (2) the Single Institution Grant. The National Library of Medicine and the Regional Medical Library programs are emphasizing the potential benefits of resource-sharing interaction where each library is made stronger by being both giver and receiver in a mutually advantageous relationship. Each type of Resource Improvement Grant is funded in two consecutive budget periods (usually one year each). An award for the first period does not automatically insure funding for the

second; application must be made for each separately. Activities supported by either kind of grant must center on providing biomedical information services to health science personnel. Programs entailing education for the general public or for patients are not eligible for grant awards. The Consortium Grant was established by NLM to promote local cooperative resource sharing. During the first budget period, the consortium as a whole is eligible to receive up to $4,000 to support the planning of organizational activities necessary to set up the consortium. In the course of this time, formal arrangements and financial agreements must be drawn up and signed by the institutional members before the consortium can apply for the second budget period funding. If the grant award is made for a second year, each institution in the consortium may receive up to $3,000 on a matching fund basis of three to one, that is, the institution matches every three dollars of grant money with one dollar of its own money. These funds are to be used by the individual libraries for the purchase of books and journals to augment resource sharing within the consortium. The Single Institution Grant is available primarily to those libraries whose geographic locations make participation in a consortium impractical. Under this kind of Resource Improvement Grant in the first budget period, a maximum of $4,000 is given to the institution to finance the development of a basic biomedical information collection in its library. The grant award for the second budget period provides up to $3,000 with a matching institutional requirement of up to $1,000 (matching fund basis of three to one). Medical Library Resource Improvement Grant application kits can be obtained from the nearest RML. One of the Medical Library Resource Improvement Program objectives is to encourage utilization of biomedical literature by supporting the development of basic information collections. Before biomedical literature can be utilized, it must be acquired, and therefore, acquisitions programs in hospital libraries are of paramount importance to the efficient functioning of the RML Network. Institutional and federal dollars must be used to optimum advantage if a solid foundation of biomedical literature is to be established at the Basic Unit level. Acquisitions as a decision-making process in a network setting is a relatively new experience for hospital librarians in general. For those who have recently entered the field of hospital librarianship, deciding which books and journals to purchase for their libraries presents 192

formidable problems, and there are few current selection guides compiled specifically for the small medical library. The "core library" concept was set forth in 1970 by Stearns and Ratcliff [1] as the basic collection necessary for a hospital library. It is minimal in scope and now notably outdated. More recent core lists have been compiled by some of the Regional Medical Libraries and Regional Medical Programs. However, because of the increased role that the hospital library is taking in its institution's clinical and educational programs, in many instances the core list is an inadequate selection guide. Likewise, book and journal lists that have been drawn up specifically for the practicing physician [2,3] do not particularly meet the hospital librarian's needs, for their coverage in some disciplines is too extensive for the Basic Unit library, while in areas such as the basic sciences, little help is provided in book and journal selection. In an attempt to bridge this gap, since 1965 I have compiled a list of books and journals especially for the small medical library, which represents a well-balanced collection including both clinical and basic science materials. While working on each biennial revision, and this is the seventh, I have always been acutely aware of current trends in hospital and other small medical libraries and have tailored my selection of books and journals to meet those demands. The present version is still primarily meant to serve as a guide in setting up and maintaining an up-to-date small medical library collection of high quality at a reasonable cost. Secondarily, it can be used as a core list for a consortium of small hospital libraries, and for its usage in this context more expensive books have been included in this list than in those preceding it. However, in instances where expensive publications are noted, less expensive alternatives are usually indicated within the same subject category. In the process of putting together this revision, I have heeded the recommendations of both biomedical librarians and subject specialists and in many cases have been guided by their suggestions when deciding what publications to include. The approximate total cost for purchasing all of the books and for entering one-year (1977) subscriptions to all of the journals in this list would amount to $18,200-$13,300 for books and $4,900 for journal subscriptions. Since many libraries cannot or may not want to buy all or even most of the listed publications, items suggested for priority purchase have been indicated by an Bull. Med. Libr. asterisk. Purchasing only the asterisked books and journals would entail an expenditure of approximately $4,500-$2,900 for books and $1,600 for one-year journal subscriptions. The, escalation in book and journal prices has not abated (Table 1). The figures shown in Table I are based exclusively on prices for publications that have been and are in this list since it was begun twelve years ago. In 1965, the average price of a journal subscription was $13.90, and in 1977, $35.14, thereby reflecting a cost increase of 152.8%. The average price of a book has risen by 87.9% from 1965 ($15.00) to 1977 ($28.18). The average book prices, especially in the 1975 and 1977 lists, are not totally representative for the clinical and basic science books, as the average has been lowered by the disproportionately large number of nursing books, which are considerably less expensive. For example, in this revision, the average price of all the books is $28.18, but when the nursing books are excluded, it jumps to $30.78, while the average price of the nursing books as a group is $12.21. I also want to emphasize the fact that the figures stated here apply only to the small medical library collection and are not indicative of the much higher book and journal costs borne by the large biomedical library. The price information quoted in this list is as accurate as possible at the time of its publication, but economic inflation will alter many of these prices within a short time if present trends continue. The selected list of books and journals given here is intended to serve as an acquisitions aid and is not set forth as the one and only definitive collection for the small medical library. No such list could meet that criterion, for each library is in-

dividual and has its unique needs. When using, this list, the hospital librarian must consider the objectives, activities, and subject specialties of the parent institution and adapt his or her selection of library materials accordingly. My emphasis has been placed on selecting the most recent authoritative works in the English language, in various price ranges where possible. The list does not represent a complete coverage of all available high quality literature in the various health science disciplines, and in most libraries it should be supplemented by additional books and journals in the subject areas of greatest interest to the library clientele. Publications of national professional associations in the health care field are generally authoritative in scope, usually inexpensive, and tend to be used heavily by hospital library patrons. A very small hospital library collection can be greatly enhanced by having these items included in it. The publication catalogs issued by these associations, for example, American Dental Association, American Hospital Association, American Medical Association, Catholic Hospital Association, or National League for Nursing are valuable acquisitions tools for the hospital librarian. The Joint Committee on Library Service in Hospitals of the CNLA has gathered together much useful information for the hospital librarian in its Basic List of Guides and Information Sources for Professional and Patients' Libraries in Hospitals [4]. All hospital librarians should be familiar with this unique reference source, which is available free of charge. Library Practice in Hospitals: A Basic Guide [5] is a book dealing with the fundamentals of health science librarianship and is directed toward the inexperienced hospital li-

brarian, but it is a worthwhile reference for anyone working in a hospital library. Its chapters on selection and acquisitions of books and journals contain basic concepts that the hospital librarian should be familiar with when using any selection aid. More detailed and advanced information can be found in comparable chapters of the Handbook of Medical Library Practice [6]. The hospital librarian should consider the Bulletin of the Medical Library Association, published quarterly, as a necessary adjunct to his or her professional development and current awareness in the field of biomedical librarianship. For health science librarians who are participating in programs for patient education, Medical Books for the Layperson: An Annotated Bibliography [7], compiled by the reference librarian from Science Reference at the Boston Public Library, is a helpful acquisitions guide. It lists 300 books published between 1969 and mid-1975 on a variety of medical and health-related subjects. I have mentioned only a few sources that can be used effectively to supplement the list of books and journals that appears on the following pages. The format of this list is similar to the previous ones. The section on Library Administration has been deleted, for I believe these publications are of far more value on the librarian's desk than in the general library collection of a hospital. In the book section, some of the clinical disciplines have been substantially increased to allow for greater selectivity within a subject field by hospital library consortia engaged in cooperative acquisitions. Books and journals that have heretofore appeared under the subject "Tumors" are now listed under the more relevant heading "Oncology." The best list of reference works for the hospital library has been compiled by Duncan; it is titled "Selected Reference Aids for Small Medical Libraries" [8]. Unfortunately, this listing is outdated, and nothing else comparable to it currently exists. In my present work, I have not tried to present a systematic coverage of reference sources for the small medical library and have noted only a few of the most obvious ones. In the book section, under Pharmacology and Therapeutics, I have included three highly technical subject reference works, namely, The National Formulary, The United States Dispensatory, and Th e Pharmacopeia of the United States of America, because in the United States they are generally the most widely recognized reference sources for pharmaceutical standards and for 194

therapeutic drug dosage. In those hospitals where these books are purchased for the pharmacy or for the pharmacology department, the necessity of duplicating them in the hospital library is questionable. Nevertheless, the hospital librarian should know where they are housed in the institution and have recourse to them for reference purposes. The book section on Dentistry has been considerably decreased in this list because recently the American Dental Association has published an annotated bibliography titled Basic Dental Reference Works [9], and Raskin and Hathorn have compiled a "Selected List of Books and Journals for the Small Dental Library" [10]. Since there is still no recent comprehensive recognized selection guide to assist nursing school or hospital librarians in their choice of book and journal materials in the field of nursing, the number of nursing books that I have recommended is purposely out of proportion to the other subject categories. For many small medical libraries, the Nursing section is far too voluminous, and the asterisked items will serve their purposes adequately. The Nursing section has been carefully revised to include current books of high caliber that deal with the most up-to-date aspects and concepts of the nursing profession. It was developed for the hospital librarian whose library supports a nursing curriculum or for the inexperienced nursing school librarian. While it is not adequate for a nursing school library, it does provide a broad base on which to build a comprehensive nursing collection. The Interagency Council on Library Resources for Nursing has published "Reference Sources for Nursing" [ I1], and I have made no effort to include nursing reference items in my current list. At the request of the Interagency Council on Library Resources for Nursing, I have agreed to compile a library selection guide for nursing books and journals within the coming year. With the significant increase in the number of hospital library consortia, both informal and formal, hospital librarians should carefully consider their individual needs for purchasing basic science materials, especially some of the journals which are very expensive. Undoubtedly, needs will vary greatly from institution to institution depending specifically upon research and educational programs. expands. Access to these journals is essential, but for many libraries, cooperative resource sharing may be the only avenue of access. For example, in order for a small medical library to purchase three of the basic science journals included in this list-Journal of Biological Chemistry, $200; American Journal of Physiology, $125; and Journal of Bacteriology, $105-it would have to expend $430 in 1977. Detailed information on cooperative serials resource sharing can be found in a paper published by Jones in 1974 [12]. In most hospital library situations, ordering books and journals directly from the publishers is an inefficient and expensive use of manpower, for it involves a massive amount of paperwork in generating and handling multiple purchase orders and invoices. A dependable medical book dealer, preferably in one's own geographic locale, and a reputable subscription agent, serving as primary sources for books and journals, can alleviate a great deal of paper work as well as many of the problems resulting from dealing with publishers directly. Both the medical book jobber and the subscription agent are above all else selling service, and the standard of service should be the determining factor when the hospital librarian chooses these vendors. The most valid information for evaluating either medical book dealers or subscription agents can be obtained from medical and academic librarians in the surrounding area who deal with them as consumers. Usually, reliable medical book dealers offer a 10% discount on books from major medical publishers in the United States. There are some medical and scientific publishers who no longer discount on retail orders sent directly to them by health science and academic libraries. While it is customary for medical book dealers to discount on the books they sell, most subscription agents now charge service fees ranging from 5 to 12%. Even so, the charge is still generally less than the expense of labor costs that result from entering subscriptions on a direct basis. A low service fee does not mean a monetary saving for a library if the agent's service is substandard; in reality it can be expensive.
